Five Priority Areas For The Australian Better Health Initiative means the following five priority areas [as explained in Annexure C (ii)(b)] within former Federal Department of Health & Ageing's  Lifescripts to help shift the focus of health care from fixing problems after they have developed, to promoting good health and reducing the burden of chronic diseases: 

  1. Promoting healthy lifestyles by encouraging people to make informed lifestyle choices and reduce the risk of developing chronic disease;

  2. Supporting early detection of risk factors and chronic disease with introduction of new Medicare Benefits Schedule item, the 45 Year Old Health Check which provides a rebate for general practitioners to undertake a health check for a patient between 45 and 49 years of age (inclusive) who is at risk of developing a chronic disease. The aim of the health check is to assist with detection and prevention of chronic disease and enable early intervention strategies to be put in place where appropriate;

  3. Supporting lifestyle and risk modification for people at high risk of developing chronic disease to make lifestyle changes and reduce their risk;

  4. Encouraging active patient self-management of chronic conditions; and

  5. Improving the communication and coordination between care services for people with chronic disease, especially cancer.
