First National Preventive Health Research Programme YELP Holistic First Business Plan YELP Holistic First Business Plan Defined Terms SWOT Analysis Executive Summary Deliverables And Costs Snapshot Page To 10 Benchmark Techniques Defined Terms for Five YELP Business Plans Second National Preventive Health Research Programme First BTAAP Business Plan Bohemian Teenagers Show Choir Programme Defined Terms BTSCP Second BTAAP Business Plan Bohemian Teenagers Symphony Orchestras Programme Defined Terms - Bohemian Teenager Symphony Orchestra Programme Third BTAAP Business Plan Bohemian Teenager Ballet & Modern Dance Programme Defined Terms BTB&MDCP Community Health Risk Factor Management Research Project - NSW means, set out on Department of Health and Ageing website regarding 'Lifestyle Prescriptions - Lifescripts', and summarised in Annexure C(ii)(b), the following project to treat 4 chronic disease risk factors, driven by NSW Department of Health - (i) a collaborative feasibility project aiming to increase the capacity of community health services to address chronic disease risk factors (smoking, poor nutrition, excessive alcohol consumption and physical inactivity) as part of their normal clinical work; (ii) funded by Centre for Chronic Disease Prevention and Health Advancement, NSW Health and managed by UNSW Centre for Primary Health Care & Equity; (iii) to produce evaluated models or approaches to integration of brief intervention for lifestyle risk factors into routine services provided by community health staff; (iv) achievements to date include the completion of a comprehensive needs assessment and the development and implementation of risk factor models in collaboration with pilot services, including development of risk factor training programs and resources for clinicians and clients including adapted “lifescripts” materials for use in the community health setting; and
(v) an evaluation of the
acceptability and uptake of risk factor models will be undertaken mid
